## Peptides: An Overview

Peptides are a big buzz word with anti-aging and fitness communities (and social media) right now - but what are they exactly?  Peptides are short chains of amino acids that serve as the building blocks of proteins. These play critical roles in many biological processes in our bodies and each chain does a little something different. 

It's important to know what's really what with peptides.Today, we’ll talk about CJC-1295/Ipamorelin, a peptide that is studied for its potential to help stimulate the body’s natural production of growth hormone (GH).

## The Hormone Cascade 

Let's upscope a sec. The body's hormone cascade begins in the hypothalamus, the brain's master command center. Think of hypothalamic hormones at the top of the chain -- like the the CEO -- and "all sh*t rolls downhill".

The hypothalamus (via releasing hormones) tells the pituitary - the primary hormone signaling gland in the brain - what to do.  The pituitary gland is like the C-Suite - comprised of different Vice Presidents, each specializing in a different function. Then, the pituitary (the VP) tells the workers (in this case, the end organs) what to do.

One of these pituitary "VPs" is called Growth Hormone or GH. Its boss in the hypothalamus is called Growth Hormone Releasing Hormone or GHRH. The end organs (workers) are the liver, stomach and fat (adipose) cells.[1] 

## What do GHRH and GH do? 

In adults, GHRH maintains and manages growth, usually in the bones, muscles and tissues. It also influences metabolism, usually in the liver through a hormone called Insulin-like Growth Factor (IGF-1) and an appetite hormone called gherlin in the stomach.[1].

Most often this communication happens via feedback loop between the pituitary and end organs. In this case, GH asks the end organs if it has enough resources to do their jobs. If the answer is "no" then it sends the feedback up the chain, where GHRH gives it permission to secrete more hormone and get the job done. If the answer is "yes" then pituitary and secreting organs holds hormone levels  steady.

As we age, the body is very slowly ramping us down over years and decades. While our levels stay fairly steady day to day, we're slowly secreting less of both hormones (GHRH and GH) over time.

## What is CJC-1295 / Ipamorelin?

CJC-1295 and Ipamorelin might be the peanut butter and jelly of peptides - better together. To understand why, let's look at each individually. 

### CJC-1295

CJC-1295 is a GHRH "analog" (i.e. it acts like it). It influences the natural release of GH from the pituitary. What's interesting is CJC's ability to increase GH levels and provide a sustained release of GH. In fact, one study showed that it can remain available in the bloodstream for an average of 6 days post injection.[2]

### Ipamorelin

Ipamorelin is a "secretagogue" that specifically tells cells to release GH. Its main target is the stomach - to release ghrelin. However, unlike other peptides similar to it (Tesamorelin, etc), Ipamorelin is less likely to influence appetite. (I always remembered ghrelin in med school as "if your stomach is growling, it must be ghrelin". Overstimulation here could make one ravenously hungry). Unlike CJC-1295, Ipamorelin is short-acting - only lasting a few hours.

Combined, CJC-1295 and Ipamorelin target and work in different areas, so they can compliment one another to maximize effect. Ipamorelin works quickly (responsible for faster initial results); while CJC is longer lived.

## Important considerations for CJC-1295/Ipamorelin

Peptides are not all roses and sunshine. They are not like over the counter supplements. Peptides exert powerful effects on the body and cannot be taken lightly. There are a number of important considerations:

### Peptides are not FDA-approved

Let me say that again - peptides are not FDA approved . And human studies are scarce. Sourcing can vary and the FDA has cracked down on the bad apples peddling "research" versions out there. A doctor trained in peptide therapies or qualified practitioner who knows reputable compound pharmacies -- and knows how to properly dose peptides -- is critical. Basically, the risks are high and   you should not do this yourself -- especially GH analogs. 

### Peptides are not for everyone

Peptides are not supplements -- they are closer to medication -- and should be prescribed by a doctor -- especially GH analogs like CJC-1295/Ipamorelin. Patients need to be screened for contraindications (pregnancy/breastfeeding, cancer, metabolic diseases, hormone imbalance and certain types of medications). Lifestyle factors need to be considered as well; it stands to reason that more active people (not couch potatoes) stand to benefit more from GH analogs like CJC 1295/Ipamorelin. 

### Patients need to be closely monitored for safe administration

Before peptides should even be an option for anyone, comprehensive labs need to be run prior to starting  peptides and regularly after that.  Key markers such as IGF-1 should be tested regularly. Protocols need to be adhered to and monitored closely. Concurrent hormone therapies, such as testosterone, need to be considered, and sometimes adjusted. Lifestyle should also be considered: for example, active people have been shown to do better with CJC-Ipamorelin vs. sedentary, where other interventions might better move the needle. 

### Ask about training and beware of "peptide mills"

Don't succumb to hype! Avoid any "Research Use Only" peptides sold online. Sourcing and product quality cannot be verified.  Understand that peptides are a gold rush right now - especially online. And like any trend, there are bad actors trying to cash in. And their ignorance can really mess you up. Beware of peptides touted in social media ads and online "wellness" companies, "research" organizations that offer direct sales to the public, and your local "med spas" and personal trainers trying to offer you peptides.

Any peptide consideration  needs to be carefully discussed with a trained practitioner - not word of mouth - based on an individual's own unique health status. And even then, any potential use needs to be well-managed. Always ask about a practitioner's training, the medical  indication  for any protocol and treatment duration and product quality. 

### Peptides should be part of an overall wellness program

Peptides like CJC-1295/Ipamorelin should not be used in isolation without careful medical consideration of one's overall health picture. Even then -- it is not a first step in any treatment not an "end all, be all"; there needs to be strong medical indications for any peptide's potential use. ### Gradual Results

The body likes gradual -- and peptide therapy is no different. Studies indicate that it can take a couple weeks or more for the body to get used to the increased hormone levels. Body composition changes have been shown to take longer, up to 3-6 months.

### Pulsed and cycled dosing

CJC-1295/Ipamorelin is typically dosed as an injection taken 5 days on, 2 days on (e.g. weekdays and weekends) for up to 3 months, depending on the doctor's protocol. Many practitioners, recommend cycling a month off to let IGF-1 levels settle. The next cycle is at the provider and patient's discretion and based on overall goals for the treatment.

 

### Side Effects

Taking CJC-1295 and Ipamorelin for too long or in high doses can lead to serious adverse effects due to prolonged, unnatural elevation of growth hormone (GH) and IGF-1 levels. These can lead to insulin resistance, hypertension, joint pain, water retention, and potential cardiovascular or cancer risks.

More day-to-day adverse effects of therapy have been shown to include headaches, redness at the injection site, skin flushing, increased hunger, restless dreams/sleep, mild fatigue or water retention. These have been shown to subside after a few days as the body adjusts to the therapy. P ersistent or severe reactions warrant immediate medical attention.

Proper storage and handling of peptides significantly impact their safety and effectiveness; poor storage can lead to other side effects from bacteria, mold, etc.

### 1. How is CJC-1295/Ipamorelin different from actual human growth hormone (HGH) injections?

CJC-1295/Ipamorelin's mechanisms stimulate the body's own natural production and release of growth hormone through the hypothalamus-pituitary pathway, whereas HGH injections provide synthetic growth hormone directly. This makes these peptides a more physiologic approach that works with your body's natural feedback loops rather than overriding them, which can be riskier.
